Mail the completed form and your check to CKCS Club of Central AZ, Inc., c/o K. Collins, 24936 N. 74th Place, Scottsdale, AZ 85255. Please make sure the form is filled out completely, including your telephone number and e-mail address.
If you need to make payment via credit card, please mail your completed form and mark the box requesting credit card payment. We will contact you to arrange this. Please be aware that if you choose this option, you will be requested to cover any handling fees associated with this (Averages $5 for the first $100 and $3 for each additional $100 of charges).
